Considerations
It is the patient's
responsibility to confirm that Dr. Moore participates in the insurance
plan.
Every effort is made to ensure the
accuracy of the participation list. The
insurance plan, IPA, PPO or the physician may initiate changes in
participation.
While we do participate in the specified
plans, the physician may not be available to all patients in a specified
plan due to plan benefits or specific plan restrictions.
It is the patient’s responsibility to
verify benefits and coverage by calling the Health Plan Administrator or
the insurance company at the number on the benefit card.
